Application for a Passport In Lieu of a Lost Passport
Required Documents
Originals, along with one set of copy on a single-sided A4 paper, are required for the following supportive documents.
1. Duly completed Form K 35 A Detailed instructions and forms are available at: https://hc-london.cwp-staging.econsulate.net/en/downloads
2. (03) identical colour photographs, Size: 3.5 cm x 4.5 cm (Please see instructions PDF) Link Sinhala / Tamil / English
3. Duly completed Form D (complaint form / statutory declaration)- this form and declaration must be certified by the Consular Division
4. Copy of the lost Sri Lankan Passport (if available)
5. Birth Certificate
o A fresh copy issued within the last six (06) months is required
o English translations alone are not accepted as originals
o In exceptional cases where the applicant’s birth certificate is unavailable, a search result certificate and the applicant’s child's birth certificate may be submitted.
6. Proof of Legal Status in the UK or the Republic of Ireland-One of the following must be provided:
o Valid visa [E-Visa Share Code (starting from “S”) / Residence Permit Card for applicants in the Republic of Ireland], or/and
o UK/Ireland/Foreign Passport (for dual citizens)
7. Affidavit (if applicable) Applicants holding Indefinite Leave to Remain (ILR), Permanent Residence (PR), or Settlement status for one year or more must submit an affidavit* confirming they have not obtained British citizenship. This affidavit must be certified by the Consular Division. Certification fee: GBP 57.00
8. Sri Lankan National Identity Card (NIC)
9. Dual Citizenship Certificate and Foreign Passport (If applicable)
10. Affidavit for applicants born outside Sri Lanka and aged over 21 years (Applicants must submit an affidavit confirming their intention to retain Sri Lankan citizenship.)
11. Affidavit for expired passports (if applicable) Applicants who remained in the UK without a valid visa or passport must submit an affidavit certified by the Consular Division.
Proof of Sri Lankan Citizenship
The following documents must be provided, if applicable:
- Dual Citizenship Certificate
- Certificate of Registration of Birth outside Sri Lanka (under section 5(2) of the Citizenship Act)
- Citizenship Registration Certificate (under Sections 8, 11, 12, or 13 of the Citizenship Act)
- Certificate of Grant of Citizenship (Indian Origin persons)
- Certificate of Grant of Citizenship (Chinese Origin persons)
Additional Requirements (If Applicable)
Inclusion of Profession in Passport
Applicants wishing to include or update their profession must provide:
- For professional occupations (Doctor, Engineer, Accountant, Quantity Surveyor, etc.): Educational certificates or professional membership proof
- For executive or skilled occupations: Employment letter from the employer (taken within 06 months)
- For unskilled occupations (ex, labour, housemaid, etc.): No supporting documents are required
- If the profession is already included in the current passport: No supporting documents required
Inclusion or Change of Husband’s Name
Married female applicants wishing to include or change their surname in the passport must submit:
- Original Marriage Certificate (Translations are not accepted as originals)
Minor Applicants (Under 16 Years of Age)
The following documents must be submitted (originals with photocopies):

4. Copy of the lost Sri Lankan Passport (if available)
5. Child’s Birth Certificate (A fresh copy issued within the last six (06) months) or Consular Birth Certificate
6. Citizenship Certificate
7. Consent from both parents (click here to download the format)
8. Parents’ valid passports (with photocopies of data and observation pages/if parents have an invalid passport, the parent’s passport renewal payment receipt must be submitted.)
9. Parents’ valid visas (E-visa share code or Residence permit card)
10. Parents’ Marriage Certificate (if married)
11. Additional documents if applicable:
o Court order (if parents are divorced, Nisi & Absolute both orders)
o Death certificate (if parent is deceased)
o Legal guardianship documents
Important: The child and both parents must be present when submitting the application.
Passport Applications for Buddhist Priests
Applicants must submit:
- Samanera Certificate, or
- Higher Ordination Certificate
