Registration of Deaths Outside Sri Lanka

Applications are accepted for the registration of deaths of Sri Lankan nationals who have passed away in the United Kingdom or the Republic of Ireland.

Please note that only deaths relating to Sri Lankan citizens or Dual Citizens of Sri Lanka may be registered through the High Commission.

Documents Required

Applicants should submit the following:

·        Duly completed application

         o   Form CR02 – where the death occurred within three months

         o   Form B 15  and Form CR02   – where the death occurred more than three months ago

·        Sri Lankan passport held by the deceased at the time of death

·        Form NQ issued by the UK Home Office,

                 An Affidavit signed by immediate family member where applicable, confirming that the deceased had not acquired British citizenship

                 at the time of death, where the deceased had been granted Indefinite Leave to Remain (ILR),

                  Permanent Residence, or Settled Status one year or earlier (This affidavit must be certified by the Consular Division. Certification fee: £57.00)

·        Death Certificate issued by the relevant authority in the country where the death occurred

·        Cremation or Burial Certificate issued by the relevant authority in the country where the death occurred

·        Original Birth Certificate of the deceased

·        Original Marriage Certificate of the deceased, if applicable

·        Original Dual Citizenship Certificate and Naturalisation Certificate, where applicable

·        Birth Certificate and Photo ID of the declarant to establish the relationship to the deceased

Important information:

·        Complete the relevant application form in BLOCK CAPITAL LETTERS.

·        CR-02 Application must be filled out in English and Sinhala or Tamil (as applicable). See instructions using the following link: Click Here

·        Applicants must submit the originals along with one set of A4-sized photocopies

·        English translations are not accepted if the original Birth Certificate or Marriage Certificate was issued in Sri Lanka.

·        If the application is submitted after one year of the date of death, the High Commission is required to obtain approval for the registration from

         the Registrar General’s Department in Battaramulla, Sri Lanka.

·        In such cases, the application will be forwarded to the Registrar General’s Department after recording the reasons for the delay.

·        Submission of forged certificates or any other false documents, and making false declarations, is an offence and will be reported to the

         relevant authorities.

Fees and Payment Details

Death Registration £19/-

*  Other applicable charges will apply.

  • Method of Payment – Cash, Debit/Credit card, Postal Order (Postal Order should be drawn in favour of the Sri Lanka High Commission)
  • Please note that personal cheques will not be accepted.
  • Fees paid will not be refunded under any circumstances.

Return of Documents

·        All original certificates will be returned after scrutiny.

·        Where documents are to be returned by post, applicants are required to provide a self-addressed prepaid Special Delivery envelope

         with postage sufficient for 500g. Applications received without such return postage will be retained until collected personally or until

         a prepaid Special Delivery envelope is provided.

·        The High Commission will not be responsible for documents lost in the post.

Collection by Representative

         Where the applicant is unable to collect the certificate/documents personally, a representative may be authorised in writing.

         The authorised person must produce an identification document and the original receipt issued by the High Commission.
